Disability Services Orientation

 

The Office of Student Disability Services has implemented a new process that will assist you during the current semester.  The changes we have made are to expedite the process so that your classroom accommodations can be processed in the most effective way.

To move forward with the Office of Disability Services orientation process please complete all of the following forms:

We strongly recommend that you familiarize yourself with the information in those documents.  You must sign all the attached documents in person.

We do not request any information on your behalf.  You must provide us with your medical records, including documentation of your diagnosis. The Release of Information Form must also be sent to  your Health Care Provider, who must release your records to the Office of Dissbility Services.

A letter alone is not enough. You will be required to provide us with a copy of your medical records.  Please send or have the information sent to Ms. Grate cgrate2@scsu.edu.  You can also have your records sent by the U. S. Postal Service to:

The orientation process will require face-to-face contact. The process will only take a few minutes, but it will give you detailed information that will clear up any misconception about your accommodations.  The final classroom accommodations will be emailed directly to you at your SC State University email address.

Again, you must sign all the documents above in person.  Once your accommodation has been signed in person, your instructors/professors will receive your accommodations in less than 48 hours of your signature.

We will strictly adhere to these procedures which will help you as the student. A copy of your information will be sent to all your instructors/professors for clarity and what they are required to do.

We believe this process will enhance your overall experience in the Office of Student Disability Services and as a student at SC State University.
